ORDINANCE NO. 79-58 Messrs. Checkley, Fahrenbach, Gaydos, Gottermeyer, Lawther. AN ORDINANCE to amend Section 345.09 of the Sanitary Code of the Codified Ordinances of the City of Lakewood, relative to the sale of frozen desserts. BE IT ORDAINED BY THE COUNCIL OF THE CITY SF LAKEWOOD, STATE OF 0HIO: Section 1. That Section 345.09 of the Sanitary Code of the Codified Ordinances of the City of Lakewood, now reading: "345.09 Application; form, Each person, firm, corporation, partnership or association desir- ing to peddle frozen desserts shall make application for a li- cense to the official charged with the enforcement of the code on forms prescribed by such official. A) Said application form shall contain~ 1. The full name and address of the applicant. 2. Complete information as to the source of supply of such frozen desserts, the location where unsold products are stored and the place where the ve- hicles or containers used for the transportation or sale of frozen desserts are stored when not in use. 3. The number of peddlers, vehicles and containers proposed to be used; and 4. Such further information relative to the business and equipment as may be deemed necessary by the official charged with the enforcement of this code. B) Each such application shall be accompanied by combined li- cense and inspection fees as follows: Three dollars ($3.00) for each peddler and three dollars ($3.00) for each vehicle or container to be operated or used in the sale of frozen desserts." shall be and is hereby amended to read as follows: 345.09 Application; form, Each person, firm, corporation, partnership or association de- siring to peddle frozen desserts shall make application for a license to the official charged with the enforcement of this code on forms prescribed by such official. A) Said application form shall 1. 2. 5 contain: The full name and address of the applicant.
