Gaydo~;"Gott~rmeyer, <br /> Law~h~r;'~Ward, <br /> Fahrenbach, Fairgrieve. <br /> <br /> AN EMERGENCYORDZNANCE aUthorizing the Mayor to fileand execute <br />an application forgroup life insurance £or the members of the Lakewood <br />Police and Fire Departments. <br /> <br /> WHEREAS, The laws Of the State of Ohio' require that'~ny group <br />life insurance plan b~ issued in the name Of tHe municipality employ- <br />ing such members of the police and fire departments, and <br /> <br /> I~HEREAS, Thisordinance constitutes an emerge~¢y~measure'for <br />the reason that it pro~idesfOr the usual daily operation of a munici- <br />pal department, now, therefore, <br /> <br /> BE IT ORDAINED BY THE CO~CIL OF THE CITY OF LAKEWOOD, STATE <br />OF OHIO: <br /> <br /> Section i~ That the Mayor b~ and he is h~reby authOri~d,'em- <br />poWered and~directed tO file and execute-an ~ppliCatiOn;~the form Of <br />which is approved by the DirectOr of Law, with any acceptable insur- <br />ance company, for a life insurance policy, which policy shall provide, <br />at no cost whatsoever to the City of Lakewood, both present and future, <br />a $ life insurance policy with all premiums to be paid <br />by the m~mSebs,'"whil~ they are active members of~the City er Lak~woad <br />Police'and Fire Departments, and with the right of eaCh'such member <br />to convert the life inSurance policy upon his termination of duty <br />with the City of Lakewood. <br /> <br /> S~ctiOn 2. That such appliCation'for a policy~hereunder, as <br />approved by the Director ~£ Law, shali abSolve'the city'of L~eWOod <br />fPom any liability aS-tO~ thepayment of premiss for sUChi~uranCe. <br />It is'agreed, hoWever; that ~h~City of Lak~wOod'~wilI mak~ the nec,' <br />essary payroll deductions for s~ch in~urance~uponreeeiving the written <br />request of the policyholder to make such deductions. <br /> <br /> The City Of Lak~wood agrees to report an~ hew applications~ <br />and terminations thereunder, provided, however; t~at failure toreport <br />any newapplicationS and termina$ionS shall'not in any way impose <br />liability upon the city for failure to do so. <br /> <br /> Section 3.
