1323.O2 APPLICATION FOR REGISTRATION a) Appl'icants for registration sha,ll apply at the office designated by the Building Commissioner and make application for registration on a form provided by the Commissioner. b) If the application is for a f~rm, partnership, corporation or other business association, the application shall contain the names and addresses of a]l members or officers, as the case may be. The application shall also contain such other relevant information as the Commissioner deems necessary. 1323.O3 TIME AND FEE a) The registration shall be valid for the calendar year in which issued and shall be renewed annually thereafter. A fee of Fifteen Dollars ($15.00) shall be paid at the time of original issuance and at each renewal thereafter. 1323.O4 INSURANCE a) Each applicant for a certificate of regist-ration sh'all furnish evidence of insurance for bodily injury in the amount of One Hundred Thousand Dollars/Three Hundred Thousand Dollars ($100,000/$300,000), and for property damages in the amount of at least Fifty Thousand Dollars ($50,000) if the applicant acts as a contractor 'for a continuous period of three months or more per calendar year. 1323.0'5 BOND a) When evidence discloses that the registrant has refused, failed or neglected to correct or abate violations of any applicable code or ordinance in performance of work done pursuant to a certificate of registration within a reasonable time after having been notified by the Building Commissioner, the Building Commissioner may require the registrant to furnish a performance bond in the amount of Five Thousand Dollars ($5,O00) guaranteeing full and faithful compliance by the applicant with all provisions of any applicable Code or Ordinance of the City of Lakewood whenever the applicant for registration named as the principal on such bond refuses, neglects or fails to correct or abate such violation within a reasonable time set by the Building Commissioner.- 1323.O6 REGISTRATION BY BUILDING COMMISSIONER a) Upon receipt of a properly, completed application form and compliance with Sections 1323.03, 1323.04 and 1323.05, the Commissioner shall register the applicant in a journal kept for that purpose, and shall issue to the regis'~rant a Certifica'te of Registration valid only for the current calendar year. The registra'tion shall expire on December '31 of each year and may be renewed from year to year. 1323.O~ REVOCATION OF REGISTRATION a) The Building Commissioner may ~isapprove or revoke any Certificate of Reglstration or renewal, thereof issued under provisions of this .Chapter for one or several of the followin9 reasons: (1) Misrepresentation of a material fact by the applicant in obtainin9 the Certificate of Registration or renewal thereof. (2) The use of the Certificate of Registration by the registrant in obtaining a Building Permit for work to be per- formed by a contractor or builder other than the registrant. (3) Violation of or non-compliance with any provision any applicab]e Code or Ordinance of the City of Lakewood the perFonm.?nce of any work done pursuant to the CertiF;c:te ssued hereunder.
