The discharge report may include, but not <br />be ! ....... d to, nature of process, rates of flow, mass discharge rate, raw <br />mm:erlai and production quantities, hours of operation, number and c!assific <br />¢ion of employees, compliance s~atus with any state or fedJral pretreatment <br />standards, or o~her information which relaxes to the generation of waste, <br />including wastewater constituents and characteristics in the wastewater <br />di~' _,.-. i_~i~ -vrts may also include the chemical constituents and <br />quan:i%v of liquid or gaseous materials stored on site, even th6ugh they <br />ma'/ not normally be discharged. At aminimum, a surm~ary of such data <br />indiaa:in~ each industrial user's compliance with this Chapter shall be <br /> <br />Prepared cuartarly and submitted to the.City Engineer. In addition =o <br />discharge reports, the City Engineer may require information in the form <br />of %.:as=ewater Discharge Permit applications, self-monitoring reports, <br />and compliance schedules. <br /> <br />SECTION 913.11: WASTe,lATER DISCHARGE PEP~IITS <br /> <br />(a) >!andatorv Permits -- All significant industrial users, or other <br />industrial users which may have a significant effect as determined by <br />the Cit~: Engineer porposing to connect or to commence a new discharge <br />to :he wastewater disposal system shall obtain a Wastewater Discharge <br />Permit before connecting to or discharging into the wastewater system. <br />All existznZ significant industrial users or industrial users subject <br />~o federal oretrea=ment standards under Section 307 (b) and (c) of the <br />Acg connected to or discharging into the wastewater disposal system <br />shall obtain a Wastewater Discharge Permit within 90 days after the <br />effective date of this Chapter. <br />(b) Pe.--m..it Amolication -- Users required to obtain a Wastewater Dis- <br />charze Pe.--m.i= shall complete and file with the City Engineer an applica- <br />uicn in the form prescribed by the City Engineer and accompanied by a fee. <br />=-,~u-~n= users shall apply for a Wastewater Discharge Permit within <br />3Q da-;s a~ter :he effective date of this Chapter, and proposed new users <br />shall apply au least 90 days prior to connecting or discharging to the <br />~asuewauer aisposal system, in support of the application, the user shall <br />suum~:, in unzts and terms appropriate for evaluation, the following <br />info.--ma=ion: <br /> <br /> i. Name, address, and loca=ion, (if different from the address); <br /> 2. SiC n~noer according to the Standard Industrial Classification <br /> l[anua~, Bureau of the Budge=, 1972, as amended; <br /> 3. Uastewater consui=uen~s and characteristics including but not <br /> limited to =hose governed by Section 250.15 of this Chapter. <br /> as determined by a reliable analytical laboratory; sampling <br /> and analysis shall be performed in accordance with procedures <br /> established by the EPA pursuant to Section 304 (~) of the Act <br /> and contained in 40 CFR, Part 136, as amended; <br /> g. Time and duration of discharge; <br /> 5. Average daily and 30 ~inute peak Wastewate~ flow rates, including <br /> daily, monthly and seasonal variations, if any; <br /> 6. Si~e plans, floor plans, mechanical and plumbing plans and details <br /> to ~how all sewers, sewer connections, and appurtenances by the <br /> ~.e,~- location and elevation; <br /> 7. Description of activities, facilities and plata processes on the <br /> nremises including all materials-which are or could be discharged; <br /> %.~ere know-n, the nature and concentration of any pollutants in <br /> discharge which are limited by any city, state or federal pre- <br /> ~reatment standards, and (for an existing discharge) a state,ant <br /> regarding whether or not the pretreatment standards are being net <br /> .on a consistent basis and if nog, whegher additional operation and <br /> maintenance and/or additional pretreatment is required for the user <br /> =o meet applicable pretrea~ent standards. <br /> 9. Each product produced by type, amount, and rate of production; <br /> 10. Type and amount of raw materials processed (average and maximum <br /> per day); <br /> 11.
