1119.15 <br /> <br />ASSISTED LIVING FACILITY OPERATING <br />S TANDA RDS o <br /> <br /> Assisted Living Facilities permitted as <br />a special exception pursuant to Section <br />1113.04 of this Zoning Code shall be operated <br />in accordance with the following operating <br />standards: <br /> <br /> (b) . . . <br /> (C) · · <br /> (d) . . <br /> (e) A plan for the supportive service <br />package which shall be included in the rental <br />agreement of residents of the Assisted Living <br />Facility, shall be submitted to the Department' <br />of Aging for review and approval prior to the <br />issuance of a Certificate of Occupancy and <br />shall include but not be limited to: <br /> <br />1. At least two meals.per day, one of <br /> which is considered the "main" meal; <br />2. Chore/housekeeping assistance; <br /> 3. Health monitoring; <br /> 4. Laundry assistance; <br /> 5. Personal support services; and <br />6. Social activities. <br /> <br />(f) . . . <br /> <br /> Section 2. It is found and determined that all formal <br />actions of this Council concerning and relating to the passage of <br />this ordinance were adopted in an open meeting of this Council, <br />and that all such deliberations of this Council and of any of its <br />committees that resulted in such formal action, were in megtings <br />open to the public, in compliance with all legal requirements <br />including Section 121.22 of the Ohio Revised Code. <br /> <br /> Section 3.
