SUGGESTED BID FORM <br /> <br /> SEALED BID FOR <br /> $ <br /> CITY OF LAKEWOOD, OHIO <br />WATER SYSTEM MORTGAGE REVENUE BONDS, SERIES 1991 <br /> <br />TO the Fiscal Officer of <br />the City of Lakewood, Ohio: <br /> <br />July , 1991 <br /> <br /> For all of the $ Water System Mortgage <br />Revenue Bonds, Series 1991 (the "Series 1991 Bonds") of the City of <br />Lakewood, Ohio (the "City"), as described in the Cityts Preliminary <br />Official Statement, dated July 16, 1991, with regard to the Series <br />1991 Bonds and in the Invitation for Bids calling for bids for the <br />Series 1991 Bonds on this date (the "Invitation for Bids", a cogy <br />of which is incorporated herein and made a part hereof), we <br />(jointly and severally, if more than one bidder is named below) <br />agree to pay the sum off ($ ) <br />representing the par value of the Series 1991 Bonds less $ <br />constituting original issue discount, if any, and $ <br />constituting underwriters~ discount, if any, plus a premium of <br />$ , plus accrued interest on the Series 1991 Bonds from <br />their date to the date of delivery, with the Series 1991 Bonds to <br />bear interest at the rates per ann~m as follows (if a fractional <br />interest rate is bid, the fraction must be 1/8 or 1/20 of 1% or a <br />whole multiple thereof; no rates shall exceed 9%) and to be retired <br />in the following principal amounts (whether at maturity or by <br />mandatory sinking fund redemption) provided that any principal <br />amount being retired by mandatory sinking fund redemption is <br />indicated by an asterisk <br /> <br />-8- <br /> <br /> <br />
