EXHIBIT A <br />Hospital Facilities <br />The following facilities to be located entirely within <br />the boundaries of the City of Lakewood will constitute Hospital <br />Facilities, the costs of which are financeable under the Act: <br />1. The Hospital Facility will be located at 12543 <br />Clifton Boulevard, Lakewood, Ohio. The Hospital Facility will <br />comprise approximately 6,200 square feet of living and nursing <br />space to be used as an intermediate care facility for the mentally <br />retarded. The Hospital Facility will include five (5) bed modules, <br />a recreation room, a nurses station, a therapy room, a living room, <br />a dining room, three (3) two-person rooms, two (2) one-person <br />rooms, bathrooms, office space, a kitchen, storage areas, staff <br />restrooms, a mechanical and electrical room, a two (2) car detached <br />garage and other equipment, furnishings and improvements. <br />
