No. 7101-95, submitted <br />originally on 12-18-95) ON 1-16-96. <br /> <br />ORDINANCE NO. ~-~-- <br /> 2-96 <br /> <br />BY:Boscia, Corrigan, Flannery, <br /> George, Seelie, Smith <br /> <br /> AN ORDINANCE authorizing and directing the Director of <br />Finance to enter into an agreement with Blue Cross (Super Blue <br />Plus and HMO), Kaiser (HMO) and Emerald Health Network (HMO) and <br />to pay from City funds the premium costs of hospitalization <br />coverage, in an amount not to exceed that appropriated by Council <br />in the 1996 Appropriation Ordinance, for persons enrolled in the <br />City's hospitalization plans for Fiscal Year 1996. <br /> <br /> WHEREAS, the Codified Ordinances of the City of <br />Lakewood authorize the Director of Finance to pay from City funds <br />the actDal premium costs of hospitalization coverage for~persons <br />enrolled in the City's hospitalization plan, and <br /> <br /> WHERF~AS, following a competitive selection process, <br />Blue Cross, Kaiser and Emerald Health Network have submitted the <br />best hospitalization packages considering cost, convenience of <br />employees and scope of coverage, and <br /> <br /> WHEREAS, it is in the best interest of the City to <br />enter into an agreement with Blue Cross (Super Blue Plus and <br />HMO), Kaiser (HMO), and Emerald Health Network (HMO) for <br />hospitalization coverage; now, therefore, <br /> <br /> BE IT ORDAINED BY THE CITY OF LAKEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That the Director of Finance, on behalf of <br />the City, is hereby authorized and directed to enter into an <br />agreement with Blue Cross (Super Blue Plus and HMO), Kaiser (HMO) <br />and Emerald Health Network (HMO) and pay from City funds the <br />actual premium costs of hospitalization coverage, for persons <br />enrolled in the City's hospitalization plan in accordance with <br />the Codified Ordinances of the City of Lakewood. <br /> <br /> Section 2. It is found and determined that all formal <br />actions of this COuncil concerning and relating to the passage of <br />this resolution were adopted in an open meeting of this Council, <br />and that all such deliberation of the Council and of any of its <br />committees that resulted in such formal action were in meetings <br />open to the public in compliance with all legal requirements. <br /> <br />Approved: <br /> <br />President <br /> <br />Cle"rk <br /> <br /> <br />
