<br />plan~ Cost allocation shall. be ,determined based on the number of employees in each <br />division participating in the hospitalization plan. The total costs of hospitalization <br />services shall be allocated according to the percent of enrollment each division <br />represents as compared to the total énrollment on the City's hospitalization pians. <br />Where there is no provision in grant programs to pay for employee hospitalization costs, <br />the Director of Finance will allocate that cost to the funds based upon the total <br />allocation of all other hospitalization costs. <br /> <br />(f) The Director of Finance is hereby authorized to pay from City funds one- <br />half of tne actual premium rate of dental insurance covérage as may be established by <br />the City for all annual salaried employee$ including Foremen Class A employees and the <br />Chiefs of the Divisions of Police and Fire but excluding all other members of the <br />Divisions of Police and Fire and full-time permanent hourly employees covered by a <br />collective bargaining agreement with the City. <br /> <br />shall be and is hereby amended to read as follows: <br /> <br />149.14 HOSPITALIZATION COVERAGE FOR EMPLOYEES. <br />(a) For the purposes of this section, "City employees" means employees of <br />Lakewood Municipal Court and the City of Lakewood, but excludes members of Council, <br />part-time hourly émplbyees, temporary employees, seasonal employees, school guards, <br />pubiic works Inspectors, and members of the Civil Service Commission. Medical benefits <br />shall become effective for each eligible employee on the first day of the calendar month <br />following the month in which the individual is hired. <br /> <br />. (b) The Director of Finance is hereby authorized to pay from City funds on <br />behalf of each City employee who is not covered by a collective bargaining agreement <br />and who is a participant in the City's primary group hospitalization plan as adopted by <br />the City, the full monthly cost of said coverage for single or family coverage basis as <br />seiected by the employee. <br /> <br />(c) The Director of Finance is hereby authorized to pay from City funds on <br />behalf of each City employee who is not covered by a collective bargaining agreement <br />and who is a participant in an authorizéd Health Maintenance Organization (HMO) the <br />full monthly cost of said coverage for single or family coverage as selected by the <br />employee providéd the full monthly cost does not exceed the amount paid by the City <br />for participants in the City's primarý group hospitalization plan. Any additional cost shall <br />be borne by the empioyee and paid for through a payroll deduction. <br /> <br />(d) The Director of Finance is hereby authorized to continue to pay to <br />employees hired prior to April 3, 1978 and have received this benefit prior to January 1, <br />2005, a cash payment in lieu of medical benefits in the following amounts: <br />1) Family Plan up to $70.26 <br />2) Single Plan up to $42.31 <br /> <br />(e) Effective January 1, 2005, the Director of Finance is hereby authorizéd to <br />pay to employees a cash payment in lieu of medical benefits as determined by the <br />Director of Finance. No employee shall be eligible for such cash payment until proof, as <br />determined adequate by the Director of Finance, is submitted establishing that the <br />employee Is covered under some other medical plan outside of the City. The cash <br />payment in lieu of medical benefits for the year 2005 shall be Seven Hundred Twenty <br />Dollars ($720.00). The Director of Finance shall annually review the cost of providing <br />medical benefits to each employee and adjust the cash payment in lieu of medical <br />benefits accordingly. <br />In the event that an employee receives a cash payment in lieu of medical <br />benefits and in the same calendar year, due to a change is status, elects to enroll in any <br />of the City's major medical plans, the employee will be required to reimburse the City a <br />prorated amount based on the number of months of enrollment in the plan for that <br />calendar year. <br /> <br />(f) Such paYfllent by the City shall be distributed among City funds according <br />to the enrollment of each division participating in the City's employee hospitalization <br />plan. Cost allocation shall be determined based on the number of employees in each <br />division participating in the hospitalization plan. The total costs of hospitalization <br />services shail be allocated according to the percent of enrollment each division <br />represents as compared to the total enrollment on the City's hospitalization plans. <br />Where there is no provision in grant programs to pay for employee hospitalization costs, <br />the Director of Finance will allocate that cost to the funds based upon the total <br />allocation of all other hospitalization costs. <br />
