RESOLUTION NO. 5478 <br /> <br />BY: Chinnock, Galla§her, Graham, McBride, <br /> Salmon, Wendling, Brown <br /> <br /> A RESOLUTION appointing Dr. Carrie Krewson Schopf as the Local <br />Registrar of Vital Statistics for the City of Lakewood District of Health, <br />effective July 7, 1980. <br /> <br />BE IT RESOLVED BY THE COUNCIL OF THE CITY OF ~AKEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That this Council, acting as the District Board of <br /> Health for the City of Lakewood,.does hereby appoint Dr.i Carrie Krewson Schopf <br />· as the Local Registrar of Vital Statistics for the City,of Lakewood District <br /> of Health in accordance with Section 3705.08 of the Ohio Revised COde, <br /> effective July 7, 1980. <br /> <br /> Section 2, That the Clerk is hereby authorized and directed to <br />forward a certified copy of this Resolution to the Ohio,Department of Health, <br />65 South Front Street, Columbus, Ohio 43215, Attention: Mr. Karl Wise, Chief, <br />Division of Vital Statistics. <br /> <br /> Section 3.
