<br /> RESOLUTION N0. 6207'87 BY: Brown, Chinnock, Gallagher, Graham, <br /> McBride, Salmon, Wendling <br /> A RESOLUTION in recognition of the fifth anniversary of the Share and <br /> Care Program and the outstanding efforts of volunteers of the Junior League of <br /> Cleveland in their service delivery. <br /> WHEREAS, volunteers of the Junior League of Cleveland provide friendly <br /> visiting, telephone reassurance, meal delivery and escort services to patients <br /> of Lakewood Hospital who are 80 years of age or older and released to their own <br /> homes, and <br /> WHEREAS, this service delivery enables a continuLUn of care from the <br /> hospital to the c~[mtunity ensuring the dignity of the patient, and <br /> WHEREAS, such services enable the patient to maintain independence <br /> within the community, and <br /> Wf~RF.AS, volunteers of_ the Junior League of Cleveland provide other <br /> related services such as public speaking and grant solicitation for the benefit <br /> of older adults, and <br /> WHEREAS, such program evolved from the joint efforts of the Junior <br /> League of Cleveland, Lakewood Hospital and the City of Lakewood, Department on <br /> Aging; now, therefore, <br /> BE IT RESOLVED BY THE COUNCIL OF THE CITY OF LAI~FIOOD, STATE OF OHIO: <br /> Section 1. That this Council extends to the Junior League of Cleveland <br /> its sincere appreciation and thanks, and that of all of the citizens of the <br /> Lakewood con¢nunity, for their dedication and tireless hours of volunteer efforts. <br /> Section 2.
