ATTACHMENT I <br /> WORK PROGRAM <br />Contract Amount $85,000 <br /> <br />The Cuyahoga County Office of Homeless Services will manage the FY <br />1995 Lakewood ESG Entitlement in conjunction with the County's own <br />Direct Entitlement. <br /> <br />The Office of Homeless Services, Cuyahoga County, will manage the <br />combined FY 1995 Shelter Grant Programs. Five percent (5%) or <br />$4,250 of the grant will be allocated to the County for their <br />administration~ <br /> <br />The County will award twenty-three percent (23%) of Lakewood's FY <br />1995 ESG allocation (or $19,550) for Essential Services/Case <br />Mansagement to the Lakewood Christian Service Center. <br /> <br />The County will award $25,500 to the Department of Human Services <br />for Homeless Prevention'activities: <br /> <br />· Emergency Housing <br />· Facing the Aids Challenge Together (F.A.C.T.) <br /> <br />$.7,176 <br /> 18,324 <br /> <br />The balance of the funds (42% or $35,700) shall be awarded for <br />operating costs of shelters for victims of domestic violence based <br />on a competitive process. <br /> <br />The Office of Homeless Services will prepare timely reports for <br />Federal agencies, monitor services provided, and conduct staff <br />Environmental Reviews prior to funding activities. <br /> <br /> <br />
