RESOLUTION NO. 7076-95 BY: Boscia, Flannery, George, Gibbons, Roth, Seelie, Smith A RESOLUTION to commend Ellen Brzytwa for her dedication and many services to the City of Lakewood. WHEREAS, the Mayor appointed Ellen Brzytwa as Chair to the Public Health Task Force and she donated thousands of hours and over a year of concentrated effort to study the status of the health of the residents of Lakewood and compared the findings with the national objectives of Healthy People 2000, and ..~ ..... WHEREAS,~.Ellen Brz~twa .provided .t. he leadership to gather together health .care professionals ~' ~' School officials and other .experts~--to facilitate a study of the City of Lakewood's public health services and .'needs, and '-~ '~ - .':'WHEREAS, as the City's advocate Mrs. Brzytwa was able to obtain professional and financial support for the Public Health Task Force recommendations, to be implemented by the year 2000, and WHEREAS, by developing, writing and publishing "The Public Health Task Force Report and Recommendations" she provided the City with a broad strategic five year plan, and WHEREAS, Ellen Brzytwa has performed above and beyond 'the call of duty in service to our community for many years as a member of the Lakewood Hospital Board of Trustees, Lakewood's St. Luke parish and many other community organizations; now therefore BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: Section 1. That the Council and the Mayor for and on behalf of its citizens extend to Ellen Brzytwa thanks and praise for her many contributions to our City. Section 2. That the Clerk of Council be and she is hereby authorized and directed to forward a certified copy of this Resolution to Ellen Brzytwa, City of Lakewood, Lakewood, Ohio 44107, and that a copy of this Resolution be spread upon the minutes of the meeting. / / Approved: PRESIDENT CLERE
