RESOLUTION NO. 7204-96 BY: Boscia, Corrigan, Flannery, <br /> George, Roth, Seelie, Smith <br /> <br /> A RESOLUTION to honor the Lakewood Department of Human <br />Services, Division of Health, on their recognition from the Ohio Department of <br />Health for their efforts in 1995 for improving childhood immunization rates. <br /> <br /> WHEREAS, the Division of Health has provided childhood <br />immunizations for the community since 1978; and <br /> <br /> WHEREAS, the Division of Health has expanded the administration <br />of childhood vaccines provided from 676 doses in 1978 to 3,108 doses in 1995; <br />and <br /> <br /> WHEREAS, the Division of Health has collaborated with area schools <br />and community organizations to achieve optimal vaccine coverage for <br />children; and <br /> <br /> WHEREAS, the Division of Health was one of six health departments <br />in the State recognized by the Ohio Department of Health for their efforts to <br />improve childhood immunization rates by having 96% of all children served by <br />the clinic receive the fourth dose of the diphtheria, tetanus and perfussis (DTP) <br />vaccine before age 2 years. This rate exceeds the Healthy People 2000 goal of <br />90% and is the highest rate of all health departments in the State of Ohio; and <br /> <br /> WHEREAS, the Division of Health continues to improve services to <br />meet the needs of all children through expansion of clinic hours including <br />evening clinics, computerization of clinic records and collaboration in a county- <br />wide registry program aimed at improving childhood immunization rates; now, <br />therefore, <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That this Council and the Mayor hereby commend the <br />Division of Health in the Department of Human Services on their recognition by <br />the Ohio Department of Health for their efforts in 1995 to improve childhood <br />immunization rates. <br /> <br /> Section 2. That the Clerk of Council be and she is hereby <br />authorized and directed to forward a certified copy of this Resolution to be <br />presented to Dorothy Buckon, Assistant Director, Division of Health, and that a <br />copy of this Resolution be spread upon the minutes of this meeting. <br /> <br />Adopted: <br /> <br />PRESIDENT <br /> <br />Approved: <br /> <br /> <br />
