RESOLUTION NO. <br /> <br />7268-97 BY: Boscia, <br /> George, <br /> <br /> A RESOLUTION authorizing and directing the t:)i.,,-ecto: of ti~e <br />Department of Human Services, on behalf of the City, to accept and <br />funds awarded by the Cleveland Clinic Foundation for the purpos:., of <br />the 'Systematic Inte~entions for a Geriatric Network of Evsk~atk.~ <br />(SIGNET)' Project. <br /> <br /> WHEREAS, the Cleveland Clinic Foundation, [.akewooo:' ,Hosp,ze-"s,~.. <br />MetroHealth Medical Center. and Kaiser Hospitals and other c;,:~mmu~dty, b~sed <br />organizations applied to and received funding from The Robc~ri Wood Jr,hr'.4or,,. <br />Cleveland and M.E. and F.J. Callahan Foundations on beheer of the:-, S GNE1 Project; <br />and .... <br /> <br /> WHEREAS, the Department of Human Services, [:~ivision of A:3in9, is <br />considered a subcontractor serving as a community-based agency to ,::¢,.m, <br /> d~,.~t the <br />elderly residing in their homes; and <br /> <br /> WHEREAS, the grant funds are to be used fc, r <br />provide the formalized linkage with the SIGNET emergency <br />practice nurses; and <br /> <br /> WHEREAS, the Department of Human Services, <br />receive $14,084.50 for the contract period of August 1, 1997, <br />31, 2000; and <br /> <br /> WHEREAS, said grant funds will be used to offsel ar~, <br />resulting in no additional expenditure of City funds; now, therefore, <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF: C)HiO <br /> <br /> Section 1.
