RESOLUTION NO. 7468-00 <br /> <br />BY: Corrigan, FitzGerald, George, <br />Seelie, Skindell, Smith. <br /> <br /> A RESOLUTION authorizing and directing the Director of the <br />Department of Human Services, on behalf of the City, to enter into an <br />Agreement with the Cuyahoga County Department of Senior and Adult <br />Services to accept and expend grant funds awarded for the purpose of <br />providing service through the OPTIONS Program. <br /> <br /> WHEREAS, the OPTIONS Program is designed to assist <br />residents who are 60 years of age or older; live in a private home or <br />apartment; have a limited monthly income; and assets, excluding home and <br />car, of 810,000 or less, by providing community-based services to assist <br />them to live and function [ndependently; and <br /> <br /> WHEREAS, the OPTIONS Program is supported by the Board of <br />County Commissioners and a three year grant of ~)1.5 million from The <br />Cleveland Foundation for this pilot program; and <br /> <br /> WHEREAS, the City of Lakewood, Department of Human <br />Services, Division of Aging, applied to the Cuyahoga County Department of <br />Senior and Adult Services to be a service provider through the OPTIONS <br />Program for the time Period covering February, 2000, through December 31, <br />2002; and <br /> <br /> WHEREAS, the Department of Human Services, Division of <br />Aging, will receive a per unit reimbursement rate for providing the following <br />services to OPTIONS clients' Home Delivered Meals, 94.85; Homemaker, <br />~ 13.43; Medical Transportation/Escort, ~9.82 and Personal Care, $13.31; <br />now, therefore, <br /> <br />OHIO <br /> <br />BE IT RESOLVED BY THE CiTY OF LAKEWOOD, STATE OF <br /> <br /> Section 1 That the Director of the Department of Human <br />Services, on behalf of the City, be and is hereby authorized and directed to <br />enter into an agreement with the Cuyahoga County Department of Senior <br />and Adult Services to receive funds for the purpose of providing home <br />delivered meals, homemaker, medical transportation/escort and personal care <br />services through the OPTIONS Program <br /> <br /> Section 2. It is found and determined that all formal actions of <br />this Council concerning and relating to the passage of this Resolution were <br />adopted in an open meeting of this Council, and that all such deliberation of <br />the Council and of any of its committees that resulted in such formal action <br />were in meetings open to the public and in compliance with all legal <br />requirements. <br /> <br /> President <br /> <br /> <br />
