R~FERRED TO FINANCE COMMITTE~ 10/16/00. <br /> <br />RESOLUTION NO. 7514-00 <br /> <br />BY: Corrigan, FitzGerald, George, Roth, <br /> Seelie, Skindell, Smith. <br /> <br /> A RESOLUTION authorizing and directing the Director of the <br />Department of Human Services, on behalf of the City, to enter into an <br />Agreement with the Cuyahoga County Department of Senior and Adult <br />Services to accept and expend grant funds awarded for the purpose of <br />providing case management and resource and referral services through the <br />COMMUNITY NAVIGATOR Program. <br /> <br /> WHEREAS, the COMMUNITY NAVIGATOR Program is designed <br />to assist grandparents and other kinship caregivers who are currently acting <br />as custodial parents for minor relatives. <br /> <br /> WHEREAS, the COMMUNITY NAVIGATOR Program is <br />supported by the Board of County Commissioners. <br /> <br /> WHEREAS, the City of Lakewood, Department of Human <br />Services, Division of Aging, applied to the Cuyahoga County Department of <br />Senior and Adult Services to be a service provider through the COMMUNITY <br />NAVIGATOR Program for the time period covering September, 2000, <br />through June 30, 2001; and <br /> <br /> WHEREAS, the Department of Human Services, Division of <br />Aging, will receive $24,1§0 for providing case management, resource and <br />referral services to eligible clients in 2000 and 2001. <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF <br /> <br />OHIO <br /> <br /> Section 1. That the Director of the Department of Human <br />Services, on behalf of the City, be and is hereby authorized and directed to <br />enter into an agreement with the Cuyahoga County Department of Senior <br />and Adult Services to receive funds for the purpose of providing case <br />management, resource and referral services through the COMMUNITY <br />NAVIGATOR Program. <br /> <br /> Section 2. It is found and determined that all formal actions of <br />this Council concerning and relating to the passage of this Resolution were <br />adopted in an open meeting of 'this Council, and that all such deliberation of <br />the Council and of any of its committees that resulted in such formal action <br />were in meetings open to the public and in compliance with all legal <br />requirements. <br /> <br />Presiden't' - <br /> <br />Mayor <br /> <br /> <br />
