RESOLUTION NO. 7546-01 <br /> <br />Corrigan, Fit3Gerald, George, Roth, <br />Seelie, Sktndell, Smith. <br /> <br /> A RESOLUTION authorizing and directing the Director of the <br />Department of Human Services, on behalf of the City, to enter into an <br />Agreement with Mental Health Services, Inc.,. to provide case management <br />services through the Children Who Witness Violence Program at a rate of <br />$75;00 per service hour. <br /> <br /> WHEREAS, the City of Lakewood, Department of Human <br />Services, Division of Youth, has been providing case management services <br />through the Children Who Witness Violence Program since May, 1999. <br /> <br /> WHEREAS, since MaY, 1999, the City of Lakewood has <br />completed two contracts for the Children Who Witness Violence Program <br />with the Board of County Commissioners in the amount of $26,299.45. <br /> <br /> WHEREAS, the Board of County Commissioners have changed <br />the procedures for receiving funding through the Children Who Witness <br />Violence Program by using PRC (Prevention, Retention and Contingency) <br />Funds and contracting with Mental Health Services, Inc. Mental Health <br />Services, Inc., has requested to contract with the City of Lakewood, <br />Department of Human Services, to be a service provider through the Children <br />Who Witness Violence Program for the time period covering February 1, <br />2001, through June 30, 2001; and <br /> <br /> WHEREAS, the Department of Human Services, Division of <br />Youth, will receive reimbursement at a rate of $75.00 per service hour for <br />providing case management to eligible clients. <br /> <br />OHIO: <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF <br /> <br /> Section 1. That the Director of the Department of Human <br />Services, on behalf of the City, be and is hereby authorized and directed to <br />enter into an agreement with Mental Health Services, Inc., to receive <br />reimbursement at a rate of $75.00 per service hour for providing case <br />management through the Children Who Witness Violence Program. <br /> <br /> Section 2. It is found and determined that all formal actions of <br />this Council concerning and relating to the passage of this Resolution were <br />adopted in an open meeting of this Council, and that all such deliberation of <br />the Council and of any of its committees that resulted in such formal action <br />were in meetings open to the public and in compliance with all legal <br />requirements. <br /> <br />President <br /> <br /> <br />
