BY: <br /> 7582-0i <br /> <br />Corrigan, George, Seelie, <br />Smith <br /> <br /> A RESOLUTION authorizing and directing the Director of the Department of <br />Human Services, on behalf of the Crty, t~ enter into an Amended Agreement with the <br />Cuyahoga County Department of Senior and Adult Services to extend an Agreement to <br />accept and expend grant funds awarded for the purpose of prov'~ing case management <br />and resource and referral services through the COMMUNTrY NAVIGATOR Program from <br />.luly 1, 2001 through .~une 30, 2002. <br /> <br />Skindell, <br /> <br /> WHEREAS, the COMMUNZTY NAVIGATOR Program is designed to assist <br />grandparents and other kinship caregivers who are currently acting as custodia! parents <br />for minor relatives; .and <br /> <br /> WHEREAS, last year, the City of Lakewood, Department of Human Services, <br />Division of Aging, received $24,150 for providing case management, resource and <br />referral services to eligible clients in 2000 and 2001 through the Board of County <br />Commissioners for the COMMUNITY NAVIGATOR Program; and <br /> <br /> WHEREAS, the City of Lakewood, Department of Human Services, Division of <br />Aging would like to extend the agreement between the Board of CoUnty Commissioners <br />for the COMMUNITY NAVIGATOR Program for an additional year in order to continue to <br />provide these valuable services to grandparent and other kinship caregivers of minor <br />relatives. Now, therefore <br /> <br />BE Ti' RESOLVED BY THE CT[Y OF LAKEWOOD,.STATE OF OHIO <br /> <br /> Section 1. That the Director of the Department of Human Services, on behalf <br />of the Cfcy, be and is hereby authorized and directed to enter into an Amended <br />Agreement with the Cuyahoga CountY Department of Senior and Adult Services to <br />extend an Agreement to accept and expend grant funds awarded for the purpose of <br />providing case management and resource and referral services through the <br />COMMUNITY NAVIGATOR Program from 3uly :t, 2001 through 3une 30, 2002. <br /> <br /> Section 2. [t is found and determined that all formal actions of this Council <br />concerning and relating to the passage of this resolution were adopted in an open <br />meeting of this council, and that all such deliberations of this Council and any of its <br />committees that resulted in such formal action, were in meetings open to the public, in <br />compliance with all legal requirements. <br /> <br />Cierk~ <br />Mayor <br /> <br /> <br />
