Lukens, M.D., Ph.D. to the <br />Emergency Medical Services Commission for the full two-year term beginning <br /> September 17, ,2001andendin§ September 17, ,2003. <br /> <br /> Whereas, Ordinance No. 31-9% adopted by Coundl July 19,199% sets forth <br />criteria for the establishment and membership of the Emergency Medical Services <br />Commission, and <br /> <br /> WHEREAS, in order to convene the Emergency Medical Services <br />Commission meetings in a timely manner with the benefit of a full Commission <br />this Resolution shall go into immediate effect upon its adoption and approval by <br />the Mayor; now, therefore; <br /> <br />BE 1T RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: <br /> <br /> Ph.D. <br /> Section 1. That Coundl hereby appoints Thomas W. Lukens, M.D, to the <br />Emergency Medical Services Commission for th~ full two-year term beginning <br />and ending at such dates determined in the preamble of this Resolution. <br /> <br /> Section 2. k ~s found and determined that all formal action of this Council <br />concerning and relating to the passage of this Resolution were adopted in an open <br />meeting of this Council, and that all such deliberations of the Council and any of <br />its committees that resulted in such formal action, were in mee~ngs open to the <br />public, in compliance with all legal requirements. <br /> <br />Adopted: <br /> <br />lVI~YOR <br /> <br /> <br />
