RESOLUTION NO. 7611-01 <br /> <br />BY: Corrigan, FitzGerald, George, <br /> Roth, Seelie, Skindell, Smith. <br /> <br /> A RESOLUTION authorizing and directing the Director of the <br />Department of Human Services, on behalf of the CitY, to enter into an Agreement <br />with Mental Health Services, Inc., to provide case management services through <br />the Children Who Witness Violence Program to be reimbursed at a rate of 918.80 <br />per service hour. <br /> <br /> WHEREAS, the City of Lakewood, Department of Human Services, <br />Division of Youth, has been providing case management services through the <br />Children Who Witness Violence Program since May, 1999. <br /> <br /> WHEREAS, since May, 1999, the City of Lakewood has completed <br />two contracts for the Children Who Witness Violence Program with the Board of <br />County Commissioners in the amount of 928,299.45. <br /> <br /> WHEREAS, the Board of County Commissioners have changed the <br />procedures for receiving funding through the Children Who Witness Violence <br />Program by contracting with Mental Health Services, Inc. Mental Health Services, <br />Inc., has requested to continue to contract with the City of Lakewood, Department <br />of Human Services, to be a .service provider through the Children Who Witness <br />Violence Program for the time period covering September 1, 2001, through August <br />31, 2002; and <br /> <br /> WHEREAS, the Department of Human Services, Division of Youth, <br />will receive reimbursement at a rate of 918.80 per service hour for providing case <br />management to eligible clients. <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That the' Director of the Department of Human Services, <br />on behalf of the City, be and is hereby aUthorized and directed to enter into an <br />agreement with Mental Health Services, Inc., to receive reimbursement at a rate of <br />91'8.80 per service hour for providing case management through the Children Who <br />Witness Violence Program. <br /> <br /> Section 2. It is found and determined that all formal actions of this <br />Council concerning and relating to the passage of this Resolution were adopted in <br />an open meeting of this Council, and that all such deliberation of the Council and <br />of any of its committees that resulted in such formal action were in meetings open <br />to the public and in compliance with all legal requirements. <br /> <br />Adopted: <br /> <br />President <br /> <br /> <br />
