RESOLU-I-~ON NO. 78o~-o3 <br /> <br />Corrigan, Dever, Dunn, <br /> George, Roth, Seelie <br /> <br /> AN EMERGENCY RESOLUTION authorizing and directing the Director of the <br />Department of Human Services, on behalf of the City, to enter into an Amended <br />Agreement with the Cuyahoga County Department of Senior and Adult Services to <br />extend an Agreement to accept and expend grant funds awarded for the purpose of <br />providing case management and resource and referral services through the <br />COMMUNITY NAVIGATOR Program through December 31, 2003. <br /> <br />FitzGerald <br /> <br /> WHEREAS, the COMMUNITY NAVIGATOR Program is designed to assist <br />grandparents and other kinship caregivers who are currently acting as custodial parents <br />for minor relatives; and <br /> <br /> WHEREAS, last year, the City of Lakewood, Department of Human Services, <br />Division of Aging, received $10,500 for providing case management, resource and <br />referral services to eligible clients through .]une 30, 2003, from the Board of County <br />Commissioners for the COMMUNITY NAVIGATOR Program; and <br /> <br /> WHEREAS, the City of Lakewood, Department of Human Services, Division of <br />Aging Would like to extend the agreement between the Board of County Commissioners <br />for the COMMUNITY NAVIGATOR Program in order to continue to provide these <br />valuable services to grandparent and other kinship caregivers of minor relatives through <br />December 31, 2003. Now, therefore <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO <br /> <br /> Section 1. That the Director of the Department of Human Services, on behalf <br />of the City, be and is hereby authorized and directed to enter into an Amended <br />Agreement with the Cuyahoga County Department of Senior and Adult Services to <br />extend an Agreement to accept and expend grant funds awarded for the purpose of <br />providing case management and resource and referral services through the <br />COMMUNITY NAVIGATOR Program through December 31, 2003. <br /> <br /> Section 2. It is found and determined that all formal actions of this Council <br />concerning and relating to the passage of this resolution were adopted in an open <br />meeting of this council, and that all such deliberations of this Council and any of its <br />committees that resulted in such formal action, were in meetings open to the public, in <br />compliance with all legal requirements. <br /> <br />Adopted: <br /> <br />Approved'~ ~ ~) ~ ~ <br /> <br />President <br /> <br />Mayor <br /> <br /> <br />
