RESOLUTION NO 7017-04 BY: CorIigan~ Demro. Dever, Dunn~ FitzGerald, Madigan, S"'eli,e. A RESOLUTION amending Resolution No. 7813~A for the purpose of increasing the authority of the Director of the Department of Human Services, on behalf of the City, to enter into an Agreement with the Cuyahoga County Board of Health to accept and expend funds for the purchase of computer equipment upgrades, cell phone and pager services, and training in the amount of $6,400 00 WHEREAS, the Federal Government set aside funding to address state and local public health infrastructure deficiencies when responding to public health emergencies; and WHEREAS, the Ohio Department of Health issued a Request for Proposals with the requirement that all health jurisdictions in each county submit a single joint proposal; and WHEREAS, representatives of the four local health jurisdictions in Cuyahoga County (Cleveland, Shaker Heights, lakewood and Cuyahoga County Board of Health) conVèl1ed the Cuyahoga County Public Health Collaborative and submitted a public health infrastructure proposal that was approved for funding for program year September 1, 2003 through August 31, 2004; and WHEREAS, lakewood's award has been increased by $6,40'0 00 to fund computer equipment upgrades, cell phone and pager services, and training; and WHEREAS, this Council by a vote of at least five (5) members elected thereto determines that this resolution is an emergency measure, and that this resolution shall take effect at the earliest date possible as set forth in ARTICLE III, SECTIONS 10 and 13 of the SECOND AMENDED CHARTER OF THE CITY OF LAKEWOOD, and that it is necessary for the immediate preservation of the public property, health and safety, and to provide the usual daily operation of municipal departments in that the project year began September 1, 2004 Now Therefore, BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO Section 1. That Resolution No 7813-03A, adopted January 20, 2004, currently reading as follows: Section 1. That the Director of the Department of Human Services, on behalf of the City, to enter into an Agreement with the Cuyahoga County Board of Health to accept and expend funds for the support of a PLlblic health nurse position in the amount of $22,845. Shall be and is hereby amended to read as follows: Section 1. That the Director of the Department of Human Services, on behalf of the City, is authorized to enter into an Agreement with the Cuyahoga County Board of Health to accept and expend funds for the support of a public health nurse position computer equipment upgrades, cell phone and pager services, and training in the amount of $29,245
