RESOLUTION NO 8385-09 BY: Antonio, Bullock, Butler, Dever, Madigan, <br />powers, Summers. <br />A RESOLUTION to take effect immediately provided it receives the affirmative <br />vote of at least five (5) members elected to Council otherwise, it shall take effect and be <br />in force after the earliest. possible period allowed by law to allow the Mayor, or his <br />designee, on behalf of the City, to enter into a Partnership Agreement with The West <br />Side Community House to co-locate Parent/Youfh Advocates and Tapestry System of <br />Care (TSOC), Care Coordinators for the period of January 1, 2010 to December 31, <br />2010 and to receive reimbursement for costs associated with providing space for those <br />positions in an amount currently estimated at $31,500.00. <br />WHEREAS, The West Side Community House will be responsible for providing <br />two Parent/Youth Advocates (1.5 FTE) and two TSOC Care Coordinators (2.0 FTE) for <br />the City of Lakewood and all related financial obligations including salary, benefits, <br />mileage, travel and professional registrations; and <br />WHEREAS, the City of Lakewood will provide supervision for the Parent/Youth <br />Advocates, office space, equipment, supplies, and overhead costs for the Parent/Youth <br />Advocates and TSOC Care Coordinators; and <br />WHEREAS, the Parent/YouttrAdvocates and TSOC Care Coordinators will be <br />integral members of the Lakewood Family to Family System of Care team, providing <br />support and advocacy for families; and <br />WHEREAS, this Council by a vote of at least five (5) members elected thereto <br />determines that this resolution is an emergency measure, and that this resolution shall <br />take effect at the earliest date possible as set forth in ARTICLE III; SECTIONS 10 and <br />13 of the SECOND AMENDED CHARTER OF THE City of Lakewood, and that it is <br />necessary for the immediate preservation of the public property, health and safety, and <br />to provide for the usual daily operation of municipal departments in that this application <br />must be submitted immediately to take advantage of these services for Lakewood <br />residents. Now, therefore <br />BE IT RESOLVED BY THE City of Lakewood, STATE OF OHIO <br />Section 1. That the Mayor, or his designee, on behalf of the City, is hereby <br />authorized to enter into a Partnership Agreement with The West Side Community <br />House to co-locate Parent/Youtli Advocates and Tapestry System of Care (TSOC), <br />Care. Coordinators for the period of January 1, 2010 to December 31, 2010 and to <br />receive reimbursement for costs associated with providing space for those positions in <br />an amount currently estimated at $31,500.00. <br />
