g. If eligible and no coupons have both been checked, the Distribution <br />Agent will then record the Applicants Waiting List: Address and a <br />phone number at which the Applicant can be reached. <br />14. Applications and Coupon Distribution Logs submitted to WRAAA: <br />a. The Distribution Agent will return the Applications and Coupon <br />Distribution Logs to WRAAA by Tuesday, July 10 and Wednesday, <br />October 70, 2012. <br />b. All coupons will be distributed by the Distribution Agent. No <br />coupons shall be returned to WRAAA. <br />c. .The Distribution Agent will make a copy of the log for their records prior to <br />submission to the WRAAA. <br />d. The Distribution Agent will report Applicants in SAMS for the program on <br />or before the tenth (10`~ calendar day of each month from July 2012 <br />through November 2012 for coupons distributed in the preceding month. <br />15. Monitoring by WRAAA <br />a. Monitoring of the Distribution Agent ahd Site is required by the ODA as <br />part of the SFMNP Program. The Distribution Agent agrees that the <br />representatives from the WRAAA shall be given full access to the <br />premises in which the program is being administered. <br />b. Tho WRAAA will conduct monitoring during normal working hours of the <br />Distribution Site with no prior notification necessary. <br />16. Record Maintenance, Accessibility and Retention <br />a. To the extent authorized by law, the Distribution Agent agrees to allow <br />representatives of the WRAAA and ODA to access all programmatic, fiscal <br />or other records rela#ed to the services for planning, auditing and <br />monitoring purposes at any time during the normal working hours of the' <br />Distribution Site with no prior notification necessary, except prior notice of <br />at least 24 hours shall be given where access is sought to the confidenfial <br />complaint files of the Distribution Site. <br />b. The Distribution Agenf agrees to keep Applicant information including but <br />not limited to: date of birth, age, sex, name, address, telephone number, <br />ethnicity, race, and proxy information (name, id number, relation, <br />telephone numberj, all found on the SFMNP Application. <br />Page 6 of 7 <br />
