Please substitute for the original. <br />RES%UTION NO. 8871 -16 BY: Anderson, Bullock, Litten, Marx, <br />Nowlin, O'Leary, O'Malley. <br />A RESOLUTION to take effect immediately provided it receives the vote of at least <br />five members of Council, or otherwise to take effect at the earliest period allowed by law, es- <br />tablishing the Foundation Planning Task Force for the purpose of creating a new Ohio nonprofit <br />corporation charged with addressing community health and wellness needs in the City of <br />Lakewood. <br />WHEREAS, Article IV of the Master Agreement among the City of Lakewood, Lake- <br />wood Hospital Association, and the Cleveland Clinic Foundation, dated December 21, 2015, <br />provides for the creation of a foundation for the purpose of addressing the wellness needs in the <br />City of Lakewood; and <br />WHEREAS, in order to form the foundation, a task force made up of residents should <br />be appointed to engage the community in the process; and <br />WHEREAS, this Council by a vote of at least five of its members determines that this <br />resolution is an emergency measure and that it shall take effect at the earliest date possible as <br />set forth in Article 1II, Sections 10 and 13 of the Second Amended Charter of the City of Lake- <br />wood and that it is necessary for the immediate preservation of the public property, health, and <br />safety and to provide for the usual daily operation of municipal departments in that the City <br />wishes to engage a task force immediately for the purposes stated above; now, therefore, <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, OHIO: <br />Section 1. The Foundation Planning Task Force is hereby created for the purpose of <br />planning the establishment of a new Ohio nonprofit corporation which will address community <br />health and wellness needs within the City of Lakewood, pursuant to the Master Agreement <br />among the City of Lakewood, Lakewood Hospital Association, and the Cleveland Clinic Foun- <br />dation, dated December 21, 2015, <br />Section 2. The Foundation Task Force shall be composed of seventeen members, seven <br />of whom shall be appointed by individual Councilmembers, two of whom shall be appointed <br />collectively by Council by separate resolution, and eight of whom shall be appointed by the <br />Mayor. The appointees shall have no formal term and may select a chairperson from among <br />them. Council may appoint a representative from among councilmembers to attend task force <br />meetings as a non - voting member, and in that event Council's representative shall report to <br />Council following each public meeting of the task force. The Mayor or any designee of his or <br />her choosing shall serve as a non - voting member. The administration shall appoint a staff <br />
