Exhibit A <br />LETTER of AGREEMENT <br />between <br />SS C &M Haven, Inc, and City of Lakewood <br />The City of Lakewood (Lakewood) agrees to assign a fill time Human Services Case Manager, <br />who will be a licensed social worker, to fill the position of Service Coordinator at Fedor Manor <br />operated by SS C &M Haven, Inc. The interview process will be conducted by both parties, The <br />selected candidate must be approved by SS C &M Haven, Inc.; such approval shall not be <br />unreasonably withheld. The position of Human Services Case Manager is within the collective <br />bargaining agreement between the City of Lakewood and the AFSCME Administrative Employees. <br />The collective bargaining agreement supersedes any and all other agreements. <br />The Service Coordinator will provide social service duties during hours, and at locations, as <br />agreed upon by SS C &M Haven, Inc. and Lakewood Department of Human Services, Division of <br />Aging. Duties are defined In the HUD Training Guidelines for Service Coordinators Notice, which <br />Is edited Into the attached Position Description, Dally administrative supervision of the Service <br />Coordinator is provided by the Fedor Manor Administrator. Clinical supervision Is provided by the <br />Lakewood Division of Aging. <br />There will be no expiration date for the Agreement. The term of the Agreement Is contingent <br />upon funding provided by grants from the Department of Housing and Urban Development <br />(HUD), If any of the grant appropriations are terminated, SS C &M Haven, Inc. will notify the City <br />of Lakewood Immediately in writing, and the contract for the related Service Coordinator as <br />covered by this Agreement will be terminated when the grant funding ends, Either party may <br />cancel this Agreement, at any time and without reason, upon 90 days written notice to the other <br />party. <br />SS C &M Haven, Inc, is responsible to reimburse for Service Coordinator related expenses, as <br />limited by the amount of the grant funding received from HUD. These expenses include salary <br />and benefits of the Service Coordinator that will be reimbursed to the City of Lakewood. The City <br />of Lakewood Is responsible to Invoice SS C &M Haven, Inc. monthly for such salary /benefits for <br />this Service Coordinator, The City of Lakewood will pay for any remaining unrelmbursed salary <br />and benefits, office rent, supplies and furniture, computer, training and travel, The City will also <br />fund the cost of the office telephone, Internet connection and the Service Coordinator's <br />professional license, <br />The Service Coordinator, along with the Lakewood Division of Aging, shall be responsible for <br />maintaining client files.
