IaAPFA4[eOAP[AgIVi ft11'tA OaalCi <br />REGIONAL <br />STORMWATER <br />onMANAGEMENT <br />PROGRAM <br />Protect Narrative <br />Community Cost -Share Program <br />Application <br />1) Project summary (1,000 word maximum) <br />Describe the Project and Include the following Information, as applicable: <br />• Describe the Project and deliverables; provide a map if applicable <br />• Submit a deliverable worksheet listing tasks and deliverables with start dates and end <br />dates for the significant benchmarks. <br />• List permitting requirements necessary to Initiate and complete project and how the <br />requirements will be met. <br />The City of Lakewood requests community cost share funds to purchase a new street sweeper to replace an existing <br />street sweeper. The new sweeper being purchased Is a 2018 Tymco -Model 600, Mounted on New 2018 International <br />Model 4300 Cab and Chassis. See the attached State of Ohio Bid. <br />The City's street sweeping program Is apart of our MS4 program Minimum Control Measure No. 6. The street <br />sweeper is one of the City's BMPs to keep debris, pollutants, and unsuitable materials out of the City's catch basins <br />and sewers. This BMP reduces the amount of TSS and other pollutants being discharged into Lake Erie and Rocky <br />River. The City's main and collector main roadways are swept once every three weeks and residential roadways are <br />swept once per month. The street sweeper Is also used after special events and to clean up accident spills and other <br />Incidents that dirty the pavements. The street sweeper Is also used monthly to clean the City's pavement at our <br />facilities, municipal parking lots, and permeable paver locations. The street sweeper utilizes water to reduce dust <br />during sweeping and to clean the surfaces. In general, the street sweepers are on dally assignment From March to <br />November, Monday through Friday. <br />The street sweeper Purchase Order would be provided to the vendor In April of 2018. The street sweeper would <br />manufactured and delivered around August of 2018. <br />Rev 5/2016 <br />
