HEALTHY URBAN TREE CANOPY GRANT PROGRAM <br />PROJECT AGREEMENT <br />Grantee: The City of Lakewood Ohio <br />Grant Title: Forest Initiative <br />Grant Award ($): $50,000 <br />Start Date of Project: 9111/2019 <br />End Date (expected): 6/3012020 <br />Signature Date of this form: 10/112019 <br />Contact person to answer questions: Jen Swallow, Chief Assistant Law Director, (216) 529 -6036 <br />INTRODUCTION <br />The 2019 Cuyahoga County Healthy Urban Tree Canopy Grant Program supports the 2019 <br />Cuyahoga County Climate Change Action Plan through active efforts to grow and maintain <br />a healthy urban tree canopy in Cuyahoga County, The grant is jointly administered by the <br />Cuyahoga County Planning Commission and the Cuyahoga Soil & Water Conservation <br />District under the auspices of the Cuyahoga County Department of Sustalnability. The 2019 <br />grant awards will fund both planning projects and implementation projects. <br />The Project Agreement provides Grant Awardees a description of the process for <br />reimbursement of project costs, and information to help ensure a successful outcome for <br />grant- funded projects.
