15.02 If a death occurs among members of the employee's family of son-in-law, daughter-in-law, <br />6 grandfather, grandmother, grandson, granddaughter, brother and sister-in-law, grandparents -in- <br />law, such employee member shall be granted five (5) days if on an eight (8) hour shift or three (3) <br />days if on a twelve (12) hour shift, of funeral leave, consecutive and contiguous to the death or <br />memorial service, without loss of pay, benefits, days off, holidays or vacation time, provided that <br />such leave may be extended within the discretion of the Chief of Police, based upon individual <br />circumstances. <br />15.03 If a death occurs to an aunt, or uncle, niece or nephew such employee member shall be <br />granted two (2) days of funeral leave, consecutive and contiguous to the death or memorial service, <br />without loss of pay, benefits, days off, holidays or vacation time. Employees will complete the <br />appropriate form in Precinct Manager. <br />15.04 Jury Duty <br />Employee members, while serving upon a jury in any court of record, shall be paid at their regular <br />salary rate for each of their workdays during the period of time so served. Time so served shall be <br />deemed active and continuous service for all purposes. In addition to the compensation provided <br />for herein, any jury fees paid to the employee/juror shall be retained by the employee. Employees <br />will complete the appropriate form in Precinct Manager. <br />15.05 Military Leave <br />Employee members shall be granted leaves of absence for military duty in accordance with state <br />and federal laws and local ordinances. <br />15.06 Leaves of absence without pay or other fringe benefits may be granted by the City at its <br />discretion. <br />15.07 Paid Parental Leave <br />Employees covered under this bargaining agreement shall be eligible for Paid Parental and Paid <br />Childbirth Leave as established through the City of Lakewood policy. <br />ARTICLE 16 HEALTH INSURANCE <br />16.01 The City agrees to provide for full-time employees and their dependents a choice of health <br />care plans, provided the City may change either carriers or delivery systems if the benefits and <br />provider networks are comparable to the present plan. The City shall not offer less than two (2) <br />plans; a PPO and HDHP option selected by the City. Prior to changing health care delivery <br />systems, the City will meet and confer with the FOP.. The PPO plan shall be a 90/10 coinsurance <br />cost share, unless unanimously decided to be changed by the Healthcare Committee. <br />16.02 The PPO plan offered by the City shall have an employee monthly contribution rate for <br />single coverage not to exceed $125.00 in 2023 and an employee monthly contribution for family <br />coverage not to exceed $245.00 in 2023.
