ORDINANCE NO. ~9-59 BY~ Messrs. Law, her, Ward, Checkley, Fahrenbach, Fairgrieve, Gaydos, Gottermeyer. AN E~RGENCY ORDINANCE authorizing the Nayer to file and execute an application for group life insurance for employees of the City of Lakewood, exclusive of the Lakewood Police and Fire Departments. WHEREA~, The laws of the State of Oh~o require that any group life insurance plan be issued in the name ef the m~icipality, as the employer ef the members of the several departments, exclusive of the police and fire departments, and ~ERE~, This ordinance constitutes an emergency measure for the reason that it provides for the usual daily operation of a munici- pal department, now, therefore, BE IT ORDAINED BY THE COUNCIL OF THE CITY OF LAKEWOOD, STATE OF OHIO: Section 1. That the Mayor be and he is hereby authorized, empowered and directed to file and execute an application, the form of which is approved by the Director of Law, with Johm Hancock Mutual Life Insurance Co. for a life insurance.policy, which policy shall provide, at no cost whatsoever to the Cmty of ~kewoed, both present and future, a $2,500,00 life insurance policy, with all premiums to be paid by the members, while they are active members of the several departments of the City of Lakeweod, exclusive of the police and fire departments; such policy shall contain a provision giving each member the right to convert the life insurance policy upon histerm- i~ation ef d~tywith the City of Lakewood. Section 2. That such application for a policy hereunder, as approved by the Director of Law, shall absolve the City of Lakewood from any liability asto the payment of premiums for such insurance. ~t is agreed, however, that ~he City of Lakewood will make the nec- essary payroll deductions for such insurance upon receiving the written request of the policyholder te make such deductions. The City of Lakewoed agrees to report any new applications ~nd terminations there~der, provided, however, that failure to report any new applications and terminations shall not in any way impose liability upon the city for failure to de se. Section 3. That this ordinance is hereby declared to be an emergemcy measure for the reasons stated in the preamble hereof, and previded it receives the affirmative vote of two-thirds of all members elected to Council, it shall take effect and be in force immediately upon its adoption and approval by the N~yor.
