ORDINANCE NO. <br /> <br />65-70 <br /> <br />BY: Messrs. Huffman, Kelley, Usher, <br /> Ward, Wendl ing <br /> <br /> AN EMERGENCY ORDINANCE authorizing and directing <br />the Director of Finance to enter into a contract with Medical <br />Mutual of Cleveland Inc. for the purpose of reducing to writ- <br />ing the existing oral agreement between the City and Medical <br />Mutual of Cleveland, Inc. <br /> <br /> wHEREAS, the City of Lakewood has been providing <br />group medical and surgical insurance benefits to eligible <br />employees of the City pursuant to an oral agreement with <br />Medical Mutual of Cleveland, Inc., and <br /> <br /> WHEREAS Medical Mutual of cleveland, Inc. has request- <br />ed that such oral agreement be reduced to Writing, and <br /> WHEREAS Mgdical Mutual of Cleveland, Inc. has submit- <br />ted to the City of~Lakewood a written Application For Group <br />Contract and Group Contract No. 612, together with Endorsement <br />21 for physical therapy which Application and Group Contract <br />are fully set forth in Exhibit "A" attached hereto and made <br />a part hereof,and <br /> <br /> WHEREAS, it is for the best interests of the City of <br />Lakewood and its employees to execute such Application, and <br /> <br /> WHEREAS, this ordinance constitutes an emergency <br />measure providing for the immediate preservation of the public <br />peace, health and safety and providing for the usual daily <br />operation of a municipal department, now, therefore, <br /> <br /> BE IT ORDAINED BY THE COUNCIL OF THE CITY OF LAKEWOOD, <br /> STATE OF OHIO: " " <br /> <br /> Section 1. That the Director of Finance of the City <br /> of LakeWood be and he is hereby authorized and directed to <br /> enter into a contract with Medical Mutual of Cleveland, Inc. <br /> to provide group medical and surgical insurance benefits.to <br /> eligible employees of the CitY of Lakewood as set forth in <br /> the Application For Group Contract and Group Contract No. 612 <br /> including Endorsement 21 for physical therapy as set forth <br /> in Exhibit "A" attached hereto and made a part hereof. <br /> <br /> " Section 2.
