ORDINANCE NO.' 66-70 <br /> <br />Messrs. Huffman, Kelley, Usher, <br />Ward, Wendling <br /> <br /> AN EMERGENCY ORDINANCE authorizing and directing <br />the Director of Finance to enter into a contract with Blue <br />Cross of Northeast Ohio for the purpose of reducing to writ- <br />ing the existing oral agreement between the City and Blue <br />Cross of Northeast Ohio. <br /> <br /> WHEREAS, the City of Lakewood has been providing <br />group hospital care benefits to eligible employees of tile <br />City pursuant to an oral agreement with Blue Cross of North- <br />east Ohio, and <br /> <br /> WHEREAS, Blue Cross of Northeast Ohio has requested that <br />such oral agreement be reduced to writing, 'and <br /> <br /> WHEREAS, glue Cross of Northeast Ohio has submitted <br /> to the City of Lakewood a written Application For Group <br /> Hospitalization Contract to become a 'part of a contract <br /> providing hospital care benefits described in the certificate <br /> captioned "730 Day Extended Benefit Employee Certificate" <br />(Form M354) which Application, together with accompanying <br />schedules,are fully set forth in Exhibit "A" attached hereto <br />and made a part hereof, ~nd <br /> <br /> WHEREAS, it is for the best interests of the City of <br />Lakewood and its employees to execute such Application, and <br /> <br />WHEREAS, this ordinance constitutes an emergency <br />measure providing for the immediate preservation of the public <br />· peace, health end.safety and providing for the usual, daily <br />operation of a municipal department,~ now, therefore, <br /> <br />BE IT ORDAINED BY THE COUNCIL OF THE CITY OF. LAKEWOOD, <br />STATE OF OHIO: <br /> <br /> Section 1. That the Director of Finance of the City <br />of Lakewood be and he is hereby authorized and directed to <br />enter into a contract with Blue Cross of Northeast Ohio to <br />provide hospital care benefits to eligible employees of the <br />City of Lakewood, as set forth in the Application For Group <br />Hospitalization Contract and accompanying schedules marked <br />Exhibit "A" attached hereto and made a part hereof. <br /> <br /> Section 2. That this ordinance is hereby declared to <br />be an emergency measure for the reasons stated in the preamble <br />hereof and providing .it receives the affirmative vot~of two- <br />thirds of all members elected to Council, it shall take effect <br />~nd be in force immediately upon its adoption and approval by <br /> <br />the Mayor. <br />Adopted: ~/~c~ <br /> <br />.7, /k/7a <br /> <br />Approved: <br /> <br />President <br /> <br />Clerk <br /> <br />Mayor <br /> <br /> <br />
