AN ORDINANCE to enter into an agreement with the District Advisory Council of Cuyahoga County, Ohio General Health District for the provision of public health services within the City of Lakewood in an amount not to exceed Two Hundred Nine Thousand Five Hundred Ninety Dollars ($209,590.00) for a one year period commencing January 1, 2009. WHEREAS, the Cuyahoga County Board of Health has proposed providing public heath services for residents of the City of Lakewood at a per capita rate of $3.70; and WHEREAS, it is necessary to continue public health services to residents of the City of Lakewood through the District Advisory Council of Cuyahoga County, Ohio General District. Now Therefore, BE IT ORDAINED BY THE CITY OF LAKEWOOD, STATE ~Je~HIC~`~~ov~izec~ j~cQ,~i~`ee`F~i~-F~ Section 1. That the Mayor, on behalf of the. City, to enter into an agreement with the District Advisory Council of Cuyahoga County, Ohio General Health District for the provision of public health services within the City of Lakewood in an amount not to exceed Two Hundred Nine Thousand Five Hundred Ninety Dollars ($209,590.00) for a one year period commencing January i, 2009 pursuant to the authority granted in Section 3709.07 of the Ohio Revised Code. Section 2. It is found and determined that all formal actions of this Council concerning and relating to the passage of this ordinance were adopted. in an open meeting of this Council, and that all such deliberations of this Council and of any of its committees that resulted in such formal action were in meetings open to the public in compliance with all legal requirements.
