PLACED ON 1~ READING & REFERRED TO THE HEALTH & <br />HUMAN SERVICES COMMITTEE SO/ZO/08, 2ND READING <br />11/3/08) <br />ORDINANCE N0. 85-08 BY: ANTONIO, BULLOCK, BUTLER, DEVER, <br />MADIGAN, POWERS, SUMMERS. <br />AN ORDINANCE to enter into an agreement with the District Advisory <br />Council of Cuyahoga County, Ohio General Health District for the provision of <br />public health services within the City of Lakewood in an amount not to exceed <br />Two Hundred Nine Thousand Five Hundred Ninety Dollars ($209,590.00) for a <br />one year period commencing January 1, 2009. <br />WHEREAS, the Cuyahoga County Board of Health has proposed providing <br />public heath services for residents of the City of Lakewood at a per capita rate of <br />$3.70; and <br />WHEREAS, it is necessary to continue public health services to residents of the <br />City of Lakewood through the District Advisory Council of Cuyahoga County, Ohio <br />General District. Now Therefore, <br />BE IT ORDAINED BY THE CITY OF LAKEWOOD, STATE ~Je~HIC~`~~ov~izec~ <br />j~cQ,~i~`ee`F~i~-F~ <br />Section 1. That the Mayor, on behalf of the. City, to enter into an <br />agreement with the District Advisory Council of Cuyahoga County, Ohio General <br />Health District for the provision of public health services within the City of <br />Lakewood in an amount not to exceed Two Hundred Nine Thousand Five <br />Hundred Ninety Dollars ($209,590.00) for a one year period commencing <br />January i, 2009 pursuant to the authority granted in Section 3709.07 of the Ohio <br />Revised Code. <br />Section 2.
