AMENDED ON THE FLOOR 9/9/09 ORDINANCE N0. 68-09 BY: Antonio, Bullock, Butler, Dever, Madigan, Powers, Summers AN ORDINANCE to take effect immediately provided it receives the affirmative vote of at least five (5) members elected to Council otherwise, it shall take effect and be in force after the earliest period allowed by law establishing. section 129.59, Vital Statistics Fees, of the Codified Ordinances of the City of Lakewood for the purpose of charging fees for Vital Statistics per chapter 3705 and 3109 of the Ohio Revised Code. WHEREAS, the State of Ohio will increase the fees of birth and death certificates by five dollars on October 16, 2009, and WHEREAS, the City of Lakewood local registrar will pass this increase and any future increases by the State to the purchaser of birth and death certificates and burial permits; and WHEREAS, this Council by a vote of at least five (5) members elected thereto determines that this ordinance is an emergency measure, and that this ordinance shall take effect at the earliest date possible as set forth in ARTICLE III, SECTONS 10 and 13 of the SECOND AMENDED CHARTER OF THE CITY OF LAKEWOOD, and that it is necessary for the immediate preservation of the public property, health and safety, and to provide for the usual daily operation of municipal departments in that this ordinance is necessary to comply with State law . Now Therefore, BE IT ORDAINED BY THE CITY OF LAKEWOOD, STATE OF OHIO: Section 1. That section. 129.59, Vital Statistics Fees, of the Codified Ordinances of the City of Lakewood is established to read as follows: 129.59 VITAL STATISTICS FEES The Director of Finance and Local Registrar are hereby authorized and directed to establish vital statistics fees for the following effective October 16, 2009: Certified Birth Certificate $25.00 Certified Death Certificate $ 25.00 Burial Permit $2.50 Said fees shall automatically increase by such amounts as fees are increased by the Ohio department of Health for said documents after the effective date of this ordinance.
