AN ORDINANCE to take effect immediately provided it receives the affirmative vote of at least five (5) members elected to Council otherwise, it shall take effect and be in force after the earliest period allowed by law, to enter into an agreement with the District Advisory Couhcil of Cuyahoga County, Ohio General Health District for the provision of public health services within the City of Lakewood in an amount not to exceed Two Hundred Nine Thousand Five Hundred Ninety Dollars ($209,590) for a one year period commencing January 1, 2011. WHEREAS, the Cuyahoga County Board of Health has proposed providing public heath services for residents of the City of .Lakewood at a per capita rate of $3.70; and WHEREAS, it is necessary to continue public health services to residents of the City of Lakewood through the District Advisory Council of Cuyahoga County, Ohio General District. Now Therefore, BE IT ORDAINED BY THE CITY OF LAKEWOOD, STATE OF OHIO: Section 1. That the .Mayor, on behalf of the City, is hereby authorized to enter into an agreement with the District Advisory Council of Cuyahoga County, Ohio General Health District for the provision of public health services within the City of Lakewood AT A PER CAPITA RATE OF $3.70 in an amount not to exceed Two Hundred Nine Thousand Five Hundred Ninety Dollars ($209,590.00) for a one year period commencing January 1, 2011 pursuant to the authority granted in Section 3709.07 of the Ohio Revised Code. Section 2. It is found and determined that all formal actions of this Council concerning and relating to the passage of this ordinance were adopted in an open meeting of this Council, and that all such deliberation of the Council and of any of its committees that resulted in such formal action were in meetings open to the public in compliance with all legal requirements. Section 3. That this ordinance is hereby declared to be an emergency measure necessary for the immediate preservation of the public peace, property, health, safety and welfare in the City and for the usual daily operation of the City for the reasons set forth and defined in the preamble to this ordinance, and provided it receives the affirmative vote of at least five (5) of its members elected to Council, this ordinance
