Referred to Health ~ Human Services Committee on 9/19/94. RESOLUTION NO. 6979-94 BY: Boscia, Flannery, George, Gibbons, Seelie, Smith A RESOLUTION approving an agreement between SS C. & M. Haven, Inc. and the City of Lakewood to assign a full-time City employee to act as service coordinator for Fedor Manor 12400 Madison Avenue. ' WHEREAS, SS C. & M. Haven, Inc. provides a unique and valuable service to the senior citizens of the City of Lakewood · through its Fedor Manor facility; and, WHEREAS, the position of Service Coordinator at this facility is necessary for the continued efficient operation of this facility; and, WHEREAS, the City desires to provide a Licensed Social Worker to act as Service. Coordinator in order to assure the continued, efficient availability of services to the senior citizens of the City of Lakewood; and, WHEREAS, SS C. & M. Haven, Inc. agrees to reimburse the City for all costs associated with this position; now, therefore, BE'IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: Section 1. That this Council hereby approves the attached agreement between SS C. & M. Haven, Inc. and the City of 'Lakewood whereby the City agrees to provide a full-time Licensed Social Worker to act as Service Coordinator at the Fedor Manor senior building with costs to the City to be reimbursed by SS C. & M. Haven, Inc. Section 2. It is found and determined that al1 formal actions of this Council concerning and relating to the passage of this resolution were adopted in an open meeting of this Council, and that all such deliberations of this Council and of any of its committees that resulted in such formal' action were in meetings open to the public and in compliance with a11 legal requirements.
