RESOLUTION NO, <br /> <br />BY: Boscia, Corrigan, Flannery, <br /> George, Seelie, Smith <br /> <br /> A RESOLUTION authorizing the Mayor, on behalf of the City, to <br />accept and expend grant funds awarded by the State of Ohio, Department of <br />Public Health, in the amount of $1,500 for the purpose of completing an <br />adolescent health needs assessment for the community. <br /> <br /> WHEREAS, the recommendations of the Public Health Task <br />Force has determined that adolescent health issues should be a' priority in <br />this community; and <br /> <br /> WHEREAS, the Department of Human Services, Division of <br />Health, has applied for and was accepted to participate in the State of Ohio's <br />Public Health Leadership Institute; and <br /> <br /> WHEREAS, a Leadership T.eam from Lakewood has been <br />convened with representatives from the Divisions of Health and Youth, <br />Lakewood Board of Education, Lakewood League of Women Voters, <br />Lakewood Hospital, Medical Profession and students; and <br /> <br /> WHEREAS, we have been notified that the Division of Health <br />has been awarded a grant in the amount of $1,500 to enable this team to <br />complete an adolescent health needs assessment for the community; and <br /> <br /> WHEREAS, said grant funds will be used to complete this <br />assessment at no cost to the City; now, therefore, <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF <br /> <br />OHIO: <br /> <br /> Section 1. That the Mayor, on behalf of the City, be and is <br />hereby authorized and directed to enter into an agreement with the State of <br />Ohio, Department of Public Health, to receive and administer grant funds for <br />the purpose of completing an adolescent health needs assessment and to <br />contain such terms and conditions as deemed necessary by the Director of <br />Law to protect the public interest. <br /> <br /> Section 2. It is found and determined that all formal actions of <br />this Council concerning and relating to the passage of this resolution were <br />adopted in an open meeting of this Council, and that all such deliberation of <br />the Council and of any of its committees that resulted in such formal action <br />were in meetings open to the public in compliance with all legal <br />requirements. <br /> <br />Adopted: ~ ~ lq4~ X~ <br /> I / President <br /> <br />Approved-'~ ~/ <br /> <br />Clerk <br /> <br /> <br />
