~y: Corrigan, FitzGerald, George, <br />~:MO[U~ON ~). 7549-01 Roth, Seelie, Sktndell, Smith. <br /> <br /> A RESOLUTION authorizing the Mayor of the City of Lakewood to enter <br />into an Agreement with FF[ Rx Managed .Care, [nc. for the provision of the <br />MatureRx Prescription Program to C'~y of Lakewood residents and their <br />immediate families. <br /> <br /> WHEREFORE, this program entitles residents to obtain discounts on <br />prescription drugs; and <br /> <br /> WHEREFORE, residents of the C~y of Lakewood who do not have <br />prescription coverage as well as those who do, could benefit from a program <br />such as that offered by FF[ Rx Managed Care, [nc. <br /> <br /> WHEREFORE, FFI Rx Managed Care, Inc. is willing to offer this <br />prescription coverage to City residents at no cost to the residents or the City of <br />Lakewood. Now, therefore, <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO: <br /> <br /> Section 1. That the Mayor of the City of Lakewood is hereby authorized <br />to enter into an agreement (a copy of which is attached hereto as Exhibit A) with <br />FFI Rx Managed Care, Inc. for the provision of the MatureRx Prescription <br />Program to city of Lakewood residents and their immediate families. <br /> <br /> Section 2. It is found and determined that all formal actions of this <br />Coundl concerning and relating to the.passage of this resolution were adopted in <br />an open meeting of this Coundl, and that all such deliberations of this Council <br />and of any of its committees that resulted in such formal action were in meetings <br />open to the public in compliance with all legal requirements. <br /> <br />Approved: <br /> <br />' PRESIDENT <br /> <br />MAYOR // <br /> <br /> <br />
