<br />RESOLUTION NO 7017-04 <br /> <br />BY: CorIigan~ Demro. Dever, Dunn~ <br />FitzGerald, Madigan, S"'eli,e. <br /> <br />A RESOLUTION amending Resolution No. 7813~A for the purpose of increasing <br />the authority of the Director of the Department of Human Services, on behalf of the City, <br />to enter into an Agreement with the Cuyahoga County Board of Health to accept and <br />expend funds for the purchase of computer equipment upgrades, cell phone and pager <br />services, and training in the amount of $6,400 00 <br /> <br />WHEREAS, the Federal Government set aside funding to address state and local <br />public health infrastructure deficiencies when responding to public health emergencies; <br />and <br /> <br />WHEREAS, the Ohio Department of Health issued a Request for Proposals with <br />the requirement that all health jurisdictions in each county submit a single joint proposal; <br />and <br /> <br />WHEREAS, representatives of the four local health jurisdictions in Cuyahoga <br />County (Cleveland, Shaker Heights, lakewood and Cuyahoga County Board of Health) <br />conVèl1ed the Cuyahoga County Public Health Collaborative and submitted a public <br />health infrastructure proposal that was approved for funding for program year <br />September 1, 2003 through August 31, 2004; and <br /> <br />WHEREAS, lakewood's award has been increased by $6,40'0 00 to fund <br />computer equipment upgrades, cell phone and pager services, and training; and <br /> <br />WHEREAS, this Council by a vote of at least five (5) members elected thereto <br />determines that this resolution is an emergency measure, and that this resolution shall <br />take effect at the earliest date possible as set forth in ARTICLE III, SECTIONS 10 and <br />13 of the SECOND AMENDED CHARTER OF THE CITY OF LAKEWOOD, and that it <br />is necessary for the immediate preservation of the public property, health and safety, <br />and to provide the usual daily operation of municipal departments in that the project <br />year began September 1, 2004 Now Therefore, <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO <br /> <br />Section 1. That Resolution No 7813-03A, adopted January 20, 2004, <br />currently reading as follows: <br /> <br />Section 1. That the Director of the Department of Human <br />Services, on behalf of the City, to enter into an Agreement with the <br />Cuyahoga County Board of Health to accept and expend funds for the <br />support of a PLlblic health nurse position in the amount of $22,845. <br /> <br />Shall be and is hereby amended to read as follows: <br /> <br />Section 1. That the Director of the Department of Human <br />Services, on behalf of the City, is authorized to enter into an Agreement <br />with the Cuyahoga County Board of Health to accept and expend funds for <br />the support of a public health nurse position computer equipment <br />upgrades, cell phone and pager services, and training in the amount of <br />$29,245 <br />
