<br />RESOLUTION NO. <br /> <br />8140-06 <br /> <br />By: . <br />Anton~o, Butler, Demro, Dever; <br />FítzGerald, Madigan, Seelie. <br /> <br />A RESOLUTION authorizing the Director of the Department of Human Services, <br />on behalf of the City, to accept funds donated by Ms. Evelyn Burri as one of the <br />beneficiaries of her Trust Estate. <br /> <br />WHEREAS, Ms. Evelyn Burri was a client of the Department of Human Services, <br />Division of Aging from November 1991 to November 2004 and received various <br />supportive services including transportation, home health and case management <br />services; and <br /> <br />WHEREAS, Ms. Evelyn Burri named the "Lakewood, Ohio Department of Human <br />Services, Division of Aging" as one of the beneficiaries of her Trust Estate according to <br />the amendment to restatement of Trust Agreement of Evelyn Burri of Lakewood, Ohio <br />(signed 2/15/02, and <br /> <br />WHEREAS, a check in the amount of $122,154.25 was disbursed to the City of <br />Lakewood Department of Human Services Division of Aging in August 2006 from the <br />final distribution of the Evelyn Burri irust;and <br /> <br />BE IT RESOLVED BY THE CITY OF LAKEWOOD, STATE OF OHIO <br /> <br />Section 1. That the Director of the Department of Human S€rvices is hereby <br />authorized, on behalf of the City, to accept funds in the amount of $122,154.25 as <br />disbursed by the Evelyn Burri Trust. <br /> <br />Section 2.
