Substituted on the floor 12/7/09 <br />RESOLUTION NO. 8381-09 gy. Antonio, Bullock, vever <br />Madigan, Powers, Summers <br />A RESOLUTION to take effect immediately provided it receives the affirmative <br />vote of at least five (5) members elected to Council otherwise, it shall take effect and be <br />in force after the earliest possible period allowed by law, authority to the Mayor, or his <br />designee, on behalf of the City, to enter into an Agreement with the Cuyahoga County <br />Department of Children and Family Services to accept and expend funds for the Family <br />to Family System of Care Expansion Project in an amount not to exceed $320,388.00 <br />for the contract period from January 1, 2010 to December 31, 2010. <br />WHEREAS, the Gity of Lakewood, Department of Human Services, will provide <br />system of care services made up of partnerships between parents, natural support <br />systems, public and private entities to implement the Vroon-Vandenberg model of <br />wraparound services as specified by Cuyahoga County; and <br />WHEREAS, the City of Lakewood, Department of Human Services, will <br />participate in a community collaborative that will act as a forum for public and private <br />agencies to make decisions about services, explore barriers to accessing these <br />services and available funding opportunities; and <br />WHEREAS, this Council by a vote of at least five (5) members elected thereto <br />determines that this resolution is an emergency measure, and that this resolution shall <br />take effect at the earliest date possible as set forth in ARTICLE Ill, SECTIONS 10 and <br />13 of the SECOND AMENDED CHARTER OF THE City of Lakewood, and that it is <br />necessary for the immediate preservation of fhe public property, health and safety, and <br />to provide for the usual daily operation of municipal departments in that this application <br />must be submitted immediately to take advantage of these services for Lakewood <br />residents. Now, therefore <br />BE IT RESOLVED BY THE City of Lakewood, STATE OF OHIO <br />Section 1. That the Mayor, or his designee, is hereby authorized, on behalf of <br />the City, to enter into a Partnership Agreement with the Cuyahoga County Department <br />of Children and Family Services to accept and expend funds for the Family to Family <br />System of Gare Expansion Project in an amount not to exceed $320,388.00 for the <br />contract period from January 1, 2010 to December 31, 2010. <br />Section 2. It is found and determined that all formal actions of this Council <br />concerning and relating to the passage of this resolution were adopted in an open <br />meeting of this Council, and that al! such deliberations of this Council and any of its <br />committees that resulted in such formal action, were in meetings open to the public, in <br />compliance with all legal requirements. <br />
