Each set of plans and specifi- <br />cations shall give the address of the work, <br />name and address of the owner of the property, <br />and of the person who prepared the plans and <br />specifications. Each application shall indicate <br />the owner of the premises, the occupant of the <br />premises and the contractor or other person to <br />be permitted to construct or erect the proposed <br />satellite dish, who shall be a licensed <br />contractor in accordance with the Codified <br />Ordinances of Mayfield Village. <br />(b) Meets all of the requirements of this ordinance. <br />(c) Submits with each application a check in the amount <br />of One Hundred Dollars ($100.00) which represents <br />a permit fee of Fifty Dollars ($50.00) and a deposit <br />fee of Fifty Dollars ($50.00) . The permit fee <br />shall cover the cost of review of the plans by the <br />Architectural Board of Review and the handling of <br />the application and inspection of the construction <br />by the Building Commissioner. The Fifty Dollar <br />($50.00) deposit shall be returnable in the event <br />the Building Commissioner, upon one inspection, <br />finds that the erection or construction is <br />acceptable. If additional inspections are required, <br />the fee shall not be returnable, and shall be used <br />to cover the cost of such further inspection. <br />1379.04 LOCATION OF SATELLITE DISHES. No satellite dish <br />shall be constructed in any front or side yard and may only be <br />constructed to the rear of the main building on the premises <br />where not visible from the street or from a neighbor's lot. <br />The plans submitted in accordance with Section 1379.03(a) shall <br />specify appropriate landscaping to reasonably conceal the dish <br />from view and the same shall be installed before approval by <br />the Building Commissioner. <br />1379.05 PENALTY. The owner of any lot or parcel where <br />anything in violation of this Chapter of the Building Code <br />is placed or exists and any architect, builder, contractor <br />or occupant who assists in the commission of any violation, <br />and any person who violates any of the provisions of this <br />Chapter of the Building Code or fails to comply therewith, <br />shall, for each violation or non-compliance, be fined not <br />more than Two Hundred Fifty Dollars (5250.00).
