2. EIVIPL®YEE TERM LIFE INSURANCE <br />Prudential will pay the amount of your life insurance to your beneficiary in the event of your death from any <br />cause. If you become totally disabled before age 60, the life insurance may be continued without premium <br />payments as long as you remain totally disabled and furnish proof of the disabilty when requested. For 31 <br />days following termination of employment, you may convert the life insurance to one of a number of <br />Prudential individual life policies without furnishing evidence of insurabilty. If you die within this 31-day <br />period, the amount of life insurance will be paid whether or not you applied for an individual life policy. <br />Amounts of term life insurance for employees age 65 and older will be 50% of those otherwise applicable <br />3. Eli/IPL®YEE ACCT®ENTAL ®EATH AN® ®ISIVIEIVIRERI~ENT <br />INSURANCE <br />Prudential will pay the amount of your AD&D insurance for your accidental loss of life or of both feet or both <br />hands or total loss of eyesight, or the loss of one hand and one foot or the sight of one eye and one hand or <br />one foot. Half amount payable for the loss of one hand or foot or the sight of one eye. The loss must result <br />from an accidental bodily injury sustained while insured. Loss must occur within 90 days of the injury. <br />Losses resulting from war, suicide, infirmity, disease, infection, or aviation except as a passenger, are not <br />covered. If Summary indicates "Off the job only," losses resulting from on the job injury will not be <br />covered. Total for one accident can't exceed the full amount of insurance. Amounts of accidental death and <br />dismemberment insurance for employees age 65 and older will be 50% of those otherwise applicable. <br />4. ®EPEN®ENT TERM LIFE INSURANCE <br />Prudential will pay you the amount of life insurance shown in the Summary in the event of your covered <br />spouse's death from any cause. Life insurance for your children 5 years or older in the amount of S 1,000 <br />will be payable to you in the event of covered child's death from any cause. Lower amounts of coverage for <br />children under 5 years but older than 13 days of age may be provided in accordance with some state laws. <br />If you die, terminate employment, or a dependent ceases to be an eligible dependent, Dependent Term Life <br />Insurance will be continued for 31 days. During this period, the dependents insurance may be converted to <br />a Prudential individual life policy without evidence of insurability. <br />5. VWEEKLY ®ISARILITV INC®~/IE BENEFIT <br />Prudential will pay the amount of your weekly benefit when you are unable to work because of sickness or <br />accidental injury (other than pregnancy) not covered by workers' compensation and requiring the care of a <br />doctor. The Summary shows the maximum period of weeks for which the benefit will be paid and the day <br />on which benefits begin. <br />10 <br />
