Important Facts <br />ENROLL NOVO/ <br />Prompt enrollment will be to your advantage. <br />To enroll, complete the enrollment form provided by your employer or Prudential representative. Enroll any <br />dependents (if dependents coverage is provided) so that they will be covered, and be sure you notify your <br />employer of any change in dependent status. <br />EFFECTIVE DATE OF COVERAGE <br />Insurance as described will be effective on the effective date of the plan provided you are actively at work on full- <br />time on that date, have satisfied any employment waiting period and enroll promptly. If you are not actively at <br />work on the effective date of the plan, the insurance will be delayed until you return to work on a full-time basis. <br />Amounts and benefits will be provided in accordance with the provisions of the plan. <br />When dependents coverage is provided, your dependents are covered when your insurance becomes effective. <br />However, if a dependent, other than a newborn child, is confined for medical care or treatment in any institution <br />or at home when coverage would normally start, the effective date of that dependent's insurance will be deferred <br />until final release by the doctor from all such confinement. <br />TERI!/IINATION OF COVERAGE <br />Coverage for you and your dependents will be terminated if: <br />You cease active work on a full-time basis, or <br />You transfer out of the eligible classes, or <br />The insurance for your class terminates, or <br />You fail to make any required contribution, or <br />The policy terminates. <br />The coverage for a dependent will also terminate if he ceases to be an eligible dependent. <br />IMPORTANT: Certain conversion or extension provisions may apply to these terminations. Your certificate will <br />provide these details. <br />This is a brief description of highlights of the plan, not a contract. The benefits are governed by the policy and <br />described in more detail in a certificate that will be issued to you when you become insured. <br />14 <br />
