Only YOU can! ~ l~r~®~ Y®ur ~er~efi~s Your new GSP Dental plan is designed to help you practice preventive dental care. It begins with a /ifetime deductib/e ...after paying only one S75 deductible, a continuously covered person will never again be required to pay a deductible charge for eligible Basic Dental Service expenses. (This helps you see your dentist every year.) And when you meet this and other specified requirements of sound preventive dentistry, your plan's incentive coinsurance feature... paying 70% in the first benefit year... increases the percentage which Prudential pays for eligible Basic Dental Service expenses. After the third consecutive benefit year in which the treatment requirements have been satisfied, Prudential pays 100% of these charges for as long as you continue to meet'the yearly requirements! (This encourages you to complete treatments promptly,) Your plan provides care for most of the dental problems you may encounter. Cleaning of teeth, filling cavities, root canal work and other preventive procedures, are considered Basic Denta/ Services. Others, including crowns, inlays, bridges and dentures (partial or full) are considered Major Dental Services. Orthodontic treatment is not covered. When certain requirements for preventive dental care are met in consecutive benefit years, the percentage is increased 10%for each year until Prudential pays 100% in the fourth year. Should the individual fail to meet these requirements in any benefit year, Prudential's share reverts to 70%.
