COUNTY OF <br />CUYAHOGA <br />BOARD OF COUNTY COMMISSIONERS <br />9-1-1 FINANCIAL ASSISTANCE PROGRAM <br />GRANT AWARD NOTIFICATION <br />----------------------- <br />Primary Entity: VILLAGE OF MAYFIELD <br />Not-To-Exceed Grant Amount: $ 1,788.50 <br />Territory Entities: VILLAGE OF MAYFIELD <br />Pursuant to the Final County-wide 9-1-1 Plan which was determined on May 27, 1986 <br />to be in effect, the aforementioned Primary Entity shall be the prim'ary public safety <br />answering point for 9-1-1 calls originating in those entities in the indicated. territory. <br />The Board of County Commissioners, on June 9, 1986, awarded the aforementioned <br />Primary Entity the indicated grant to financially assist the Primary Entity with the <br />initial cost of installing that equipment at the primary public safety answering point <br />necessary to utilize the automatic number identification and the automatic location <br />identification features of the Enhanced 9-1-1 System. <br />The acceptance of this ;grant shall only indicate the Primary Entity's intent to install <br />the automatic number identification and the automatic location identification equipment <br />at its primary public safety answering point. To receive, funds under this grant, the <br />Primary Entity must enter into a contract with the Board of County Commissioners and <br />must submit evidence that the automatic number identification and . the automatic <br />location identification equipment has been installed at the primary public safety <br />answering point and that the initial cost of this equipment has been paid by the Primary <br />Entity. . <br />To indicate the aforementioned Primary Entity's acceptance of this grant, this Grant <br />Award must be signed by an authorized representative of the Primary Entity and <br />returned by 4:30 P.M. on Friday, September 12, 1986 to Melvin B. Mixner, MAPS Group, <br />Board of County Commissioners, 415 County Administration.
