RESOLUTION N0. 83-1!} <br />Page -2- <br />WHEREAS, both the Planning & Zoning Commission and the <br />Council of Mayfield Village nave previously determined that the <br />continued use of the subject premises as a general dental practice is <br />in the best interests of the health, safety and welfare of the <br />residents of Mayfield Village, Ohio; and <br />WHEREAS, the Planning & Zoning Commission of Mayfield <br />Village, Ohio, has further aetermined that Dr. Jeffrey Eadie should be <br />permitted to continue the operation of a general dental practice from <br />the subject premises conditioned upon the restrictions as hereinafter <br />set forth; now, therefore, <br />BE IT RESOLVED BY THE COUNCIL OF HAYFIELD VILLAGE, OHIO, THATs <br />Section 1. Pursuant to the approval granted by the <br />Planning & Zoning Commission, the Council of Mayfield Village, Ohio, <br />hereby grants to Dr. Jeffrey Eadie a U-6 Permit to use the premises <br />located at 865 S.O.M. Center Road, Mayfield Village, Ohio, for the <br />purpose of conducting a general dental practice. <br />Section 2. This Permit shall terminate if and when Dr. <br />Jeffrey Eadie ceases to conduct a full-time general dental practice <br />from the subject premises. <br />Section 3. During the term of the U-6 Permit granted <br />herein the use of the premises shall be restricted to two (2) dental <br />practitioners operating on the premises at any one time. <br />Section 4. At such time as Dr. Jeffrey Eadie vacates the <br />subject premises, he shall provide written notice to any subsequent <br />tenant, or to the owner of the premises if different from himself, <br />that the subject premises is zoned for single tamily residential use <br />and that upon his vacation of the premises, the Permit granted herein <br />shall expire and the property shall revert to single tamily residen- <br />tial use. <br />Section 5._ All prior resolutions inconsistent with this <br />resolution in whole or in part are repealed to the extent necessary to <br />avoid conflict with this resolution. <br />Section 6. That the Council finds and determines that all <br />formal actions of this Council relating to the adoption of this <br />Resolution have been taken at open meetings of this Council; and that <br />deliberations of this Council and of its committees, resulting in such <br />formal action, took place in meetings open to the public, in <br />compliance with all statutory requirements, including the requirements <br />of Section 121.22 of the Ohio Revised Code. <br />
