Three (3) copies of the floor plan, <br />approved by the Building Commissioner/Ser- <br />vice Director which shall include <br />construction or alteration and <br />specifications necessary for the <br />change of occupancy. <br />3. Name, address and telephone number of <br />the person who is to be contacted for <br />emergency purposes including, but not <br />limited to, alarm companies, if appli- <br />cable. <br />4.- An endorsement by the Mayfield Village <br />Fire and Police Departments that all <br />Village safety requirements have been met. <br />5. A listing of any hazardous materials used <br />or stored per Title 3, Superfund Amend- <br />ments and Reauthorization Act of 1986 <br />(SARA). <br />6. In case of the manufacturing and/or <br />storage of hazardous materials, TIER 1 <br />and TIER 2 forms as required by SARA. <br />7. The number of employees as defined for <br />taxation purposes by the Regional Income <br />Taxation Agency. <br />(c) The Building .Commissioner/Service Director shall <br />not issue an occupancy certificate until an <br />inspection of the premises is completed in <br />accordance with the floor plan and specifica- <br />tions filed and approved, until all necessary <br />fees for building permits have been paid to the <br />Village, and until all of the provisions of this <br />Part Thirteen - Building Code and other applicable <br />Ordinances of the municipality have been complied <br />with. <br />SECTION 2. All prior Ordinances inconsistent with this Ordinance in <br />whole or in part are hereby repealed to the extent necessary to avoid conflict <br />with this Ordinance. <br />
