Ordinance No. 1083 <br />(2) <br />Section 3. Other Papers to Accompanv Application for Permit. The <br />following papers shall accompany the Application for a Permit under this <br />Ordinance: <br />%, <br />(a) A cash bond in the amount of $1,000.00, or such larger amount as <br />the Building Inspector may, in writing, require as in his judgment being <br />necessary for the protection of public and private property, shall be deposited <br />with the Village Clerk-Treasurer. Said deposit shall be refunded to the person <br />making the same upon proof of his compliance with all of the Ordinances of the <br />Village of Mayfield and other laws applicable to the performance of such work <br />and furthei provided that he has compensated the Village of Mayfield and any <br />other person or persona entitled to such payment for all property damage caused <br />by the person making such deposit in the performance of such work, and this ap- <br />plies to any public damage to public roads; otherwise, the Village may deduct <br />from the amount of the cash deposit the reasonable cost, as determined by the <br />Building Inspector, of repairing such damage and shall return to the permit <br />holder any unexpended balance. If the cost of repairing such roads or property <br />is in excess of the amount of the cash deposit, the permit holder performing <br />such work shall nevertheless remain liable to the Village or the property owner <br />or owners, as the case may be, for any excess damage. <br />(b) The permit holder shall carry liability insurance, protecting <br />the Village of Mayfield against liability in the amount of $100,000.00 as to any <br />one person and $300,000.00 as to all persons by reason of personal injuries and <br />death, and in the amount of $25,000.00 by reason of damage to property caused <br />by the work authorized by the permit. The Building Inspector may require the <br />permit holder to deposit said policy or proof thereof with him, and it shall <br />name the Village of Mayfield as an additional assured. <br />(c) If the building is located within the Village of Mayfield, there <br />shall be filed with-the application for a permit sufficient evidence that the <br />building and the lot from which it is to be removed, are free from encumbrances <br />and that all current taxes and assessments, both general and special, against <br />the same, have been paid in full. The applicant, if he is not the owner of the <br />premises from which the building is to be moved, shall file with the application <br />a written statement or bill of sale signed by the owner, or other sufficient <br />evidence that he is entitled to remove the building. <br />(d) Where utility companies supplying service to the Village will be <br />affected, the applicant shall file with his application suitable letters of <br />clearance of such utility companies consenting_to the removal. It is deemed <br />that the utilities will be affected except when the removal is limited to a re- <br />location on the same or an abutting lot and no portion of any Village street <br />will be entered upon, <br />Section 4. Approval of Application by Building Inspector. <br />(a) Inspection Period: The Building Inspector shall inspect the build- <br />ing and the applicant's equipment to determine whether the standards for issuance <br />of a permit are met. <br />(b) Standards for Issuance: The Building Inspector shall issue a <br />permit for the moving of such building if he finds upon inspection that all of <br />the following requirements are met: <br />i <br />
